You will collaborate with physicians and the care team, contribute to discharge planning, develop individualized plans of care, and ensure safe, cost-efficient care that centers on residents and families.

Qualifications

• Licensed RN in the state of practice
• Must hold and maintain CPR certification, or obtain within 90 days of hire
• No findings of abuse, neglect, or misappropriation in the state registry

Responsibilities

• Deliver quality nursing care focused on outcomes using the nursing process
• Assist with discharge planning and follow-up throughout length of stay
• Collaborate with residents, families and interdisciplinary care team to develop individualized care plans
• Implement plans consistently to achieve resident-centered outcomes while using available resources
• Share information with physicians and other health care professionals to update care plans
• Maintain resident comfort and knowledge of current nursing practices
