Job summary

Signature HealthCARE in Elizabethtown, KY is seeking an RN to deliver compassionate nursing care within a supportive team. You’ll administer medications, assist with ADLs, and help residents feel safe, dignified, and well cared for across our long-term care settings.

Active RN license in the state, CPR/BLS preferred, and at least one year of clinical nursing experience (long-term care preferred) are desired.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
