Qualifications

• Maine RN licensure required.
• Three years post-licensure nursing experience in community, family, pediatric, hospital, or psychiatric care preferred.
• Ability to navigate electronic health records and generate data summaries.

Responsibilities

• Engage, assess, educate, coordinate, and intervene to improve care and outcomes.
• Conduct in-person visits in the community, homes, hospitals, or clinics.
• Collaborate with a multidisciplinary team and document care in the EHR.
• Support ISP development and monitor nursing care.
