Electra Hospital is seeking a Community Health & Wellness Coordinator (RN) to deliver low- or no-cost health screenings, education, preventive services, and care coordination within the community. The role emphasizes diabetes management, obesity prevention, and collaboration with providers to improve population health across the service area.

The program requires nursing care delivery across the lifespan, class facilitation, data tracking, and outreach activities.

Qualifications

• RN license in Texas required.
• BSN preferred.
• Minimum two years of clinical nursing experience.
• Experience in primary care, family practice, chronic disease management, community health or rural healthcare preferred.
• Strong assessment and clinical decision-making skills.
• Excellent verbal and written communication skills.

Responsabilités

• Provide nursing services, health education, preventive care, screenings, and care coordination across the lifespan.
• Conduct low- or no-cost chronic disease screenings per clinical protocols.
• Assess, treat, and manage acute and chronic health conditions within RN scope.
• Develop individualized care plans addressing physical, behavioral, nutritional, and lifestyle factors.
• Deliver evidence-based diabetes management services and care coordination.
• Host and facilitate community health and wellness classes focused on prevention and healthy living.
