Job summary

The Primary Health Network is seeking a Community Health Nurse to promote wellness through education, screenings and direct care. This role focuses on reducing health disparities by addressing environmental, physical, and social determinants of health in the community.

You will collaborate with diverse populations, provide immunizations per CDC/CDPH guidelines, and contribute to outreach and advocacy initiatives while maintaining comprehensive patient-centered care.

Qualifications

• Graduate of an approved school of nursing.
• Current unrestricted PA and Ohio RN license.
• Knowledge and ability to provide routine immunizations per CDC/CDPH guidance.

Responsibilities

• Promote wellness through education, screenings and care delivery.
• Address health disparities in marginalized communities through outreach and advocacy.
