Job summary

Option Care Health, Inc. in Riverside, CA, is seeking a Clinical Transition Specialist, an experienced sales professional who educates patients, families and facility staff about home health services and products, and drives successful transitions from hospital to alternative site care.

You will build and maintain relationships with referral sources, collaborate with Account Managers to grow referrals, coordinate pre-admission information, ensure timely discharge, and support a seamless patient

Qualifications

• Must establish and maintain relationships at all organizational levels.
• Experience delivering customer service to internal/external customers.
• Knowledge of ACHC/URAC standards is preferred.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Interacts with area service providers to sell Option Care services and assess patient suitability.
• Uses sales techniques to educate patients, families and facility staff about the services.
• Provides hands-on, in-person education to patients and caregivers for timely discharge.
• Communicates with agency staff, medical team, patients and family throughout discharge to implement a treatment plan.
• Responsible for collecting, reviewing and completing pre-admission information and signoff.
• Partners with Account Manager and Regional Sales Director to create and execute area business and growth plans.
• Coordinates with marketing to deliver educational programs to patients, caregivers and referral sources.
• Reviews medical records to obtain history and insurance payor information and shares with registration.
• Initiates care transition coordination with referral sources and internal partners to ensure seamless transition.
