Qualifications

• Active and unrestricted RN license required.
• Minimum of 2 years in healthcare.
• Experience establishing relationships across organizational levels.
• Knowledge of ACHC and URAC standards.
• Customer service experience with internal and external customers.

Responsibilities

• Educate and train patients, caregivers, and facility staff on OCH services and products for successful transition to home or alternate sites.
• Conduct patient assessments to determine eligibility for the OCH care model.
• Coordinate with sales to identify educational needs and improve patient transition volume.
• Maintain confidentiality of patient information per legal guidelines.
• Document education using OCH applications and technology.
• Coordinate patient transitions from hospital to home or alternate care settings with facility staff and providers.
