Job summary

Elara Caring is hiring an RN Clinical Team Manager to oversee patient care services, coordinate care plans, and ensure regulatory compliance within our home health operations. The role requires a current RN license and proven OASIS experience, with supervisory responsibilities and travel within the branch area.

The position offers autonomy, direct patient care collaboration, and opportunities for leadership across the home health team.

Qualifications

• RN license required.
• Skilled Home Health/OASIS experience as RN required.
• Associate degree in Nursing or related field required.
• Ability to travel within branch/office coverage area.
• Proficiency with Homecare Homebase, Workday and MS Office preferred.

Responsibilities

• Oversee patient care services and personnel, assign patients and coordinate care.
• Direct delivery of home health services and coordinate with providers and facilities.
• Ensure timely start of patient care within regulatory guidelines.
• Coordinate physician orders, staff assignments, and communications with patients and families.
• Review and approve plan of care and changes for clinical appropriateness.
• Conduct regular OASIS reviews for all patients and provide staff guidance.
