Elara Caring in Bloomington, IN is seeking an experienced RN Clinical Team Manager to lead patient care teams in home health. You will oversee care delivery, coordinate referrals, manage staff assignments, and ensure plans of care meet regulatory standards.

The role requires an Associate degree in nursing, 2 years home health RN experience, and 1 year of supervisory experience. Travel within the branch area, a valid driver's license, and ability to lift up to 50 pounds are required.

Qualifications

• Associate degree in a Nursing related field is required.
• 2 years home care experience as a registered nurse in a Home Health or Hospice environment.
• 1 year of supervisory experience preferred.
• Current, unrestricted RN license valid for the state of work.
• Ability to travel within branch/office coverage area with a valid driver’s license.

Responsabilités

• Oversee patient care services and personnel including assignments and care coordination.
• Plan, direct and participate in delivery of home health services and referrals.
• Ensure timely start of patient care within regulatory guidelines.
• Coordinate physician orders, staff assignments and communications with families.
• Review and approve plan of care and integrate orders from physicians.
• Conduct regular OASIS reviews and provide guidance to the team.
• Initiate hiring requests, interview candidates and recruit staff.
• Communicate with Branch Director on key performance metrics.
