Elara Caring is seeking a Clinical Team Manager to lead and coordinate home health services, ensuring quality patient care and regulatory compliance. You will supervise staff, review plans of care, and drive performance in collaboration with the Branch Director.

Qualifikationen

• Associate degree in a Nursing related field required.
• 2 years home care experience as a RN in Home Health or Hospice
• 1 year supervisory experience preferred
• Current, unrestricted RN license valid for the state of work
• Proficiency with Homecare Homebase, Workday, and Microsoft Office

Aufgaben

• Oversee patient care services, staff assignments, referrals, and care coordination to ensure patient needs are met.
• Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.
• Ensure timely start of care and regulatory compliance.
• Review and approve Plans of Care and physician orders.
• Conduct regular OASIS reviews to support documentation and quality care.
• Provide coaching to direct care team and assist with interviewing and hiring clinical staff.
• Collaborate with Branch Director to monitor performance metrics and support clinical operations.
