Job summary

AccentCare, Inc. is seeking a Hospice Registered Nurse Team Director (Full-Time) in Killeen, TX. The role leads the clinical team, oversees care planning, and coordinates interdisciplinary collaboration across Bell and Hamilton counties.

The position requires an RN license and supervisory experience in hospice or related fields, with CHPN preferred and willingness to travel as needed.

Qualifications

• ADN or higher required.
• Registered Nurse license in state(s) practicing, required.
• Three+ years' hospice/home health/general medical/surgical experience.
• Minimum of one year supervisory experience.
• CPR certification preferred.
• Must be a licensed driver able to travel to all locations.
• Meet state regulatory requirements.

Responsibilities

• Supervise direct patient and family care team members with site leadership.
• Handle scheduling, payroll, and administrative duties.
• Ensure timely initial assessments and required reassessments by qualified staff.
• Oversee IDG care plan development and review for each patient/family.
• Collaborate with site leadership to manage staff performance and documentation.
• Promote Back to Bedside and case-management skills per protocol.
• Support staff education and training initiatives.
• Facilitate IDG meetings and provide triage and caller support.
• Contribute to quality improvement through data collection and reporting.
• Maintain clear internal and external communications.

What You Need to Know

Hospice Registered Nurse Team Director Qualifications

• ADN or higher required
• Registered Nurse license in state(s) practicing, required
• Certified Hospice Nurse (CHPN) preferred
• Three+ years' experience in hospice, home health, general medical surgery, oncology, or patient population-specific nursing (i.e., geriatrics, pediatric, etc.)
• Minimum of one year of supervisory experience
• Valid CPR certification while employed in a clinical role (preferred)
• Must be a licensed driver who can travel to all business locations
• Meet the regulations and requirements of the state(s) in which the program provides services

Hospice Registered Nurse Team Director Responsibilities

• Supervise direct patient and family care team members, in collaboration with program site leadership
• Scheduling, payroll, and other administrative duties
• Ensure all initial assessments and required reassessments are conducted by qualified team members within required time frames, including documentation submissions, and/or synchronization
• Oversee and support the Interdisciplinary Group (IDG) in development, implementation, and review of the interdisciplinary plan of care for each patient and family
• Collaborate with program site leadership members to monitor, manage, and mentor staff in patient care activities, productivity, and documentation per related policies and protocols
• Participates and promotes Back to Bedside efforts in reinforcing Case Management skills, and knowledge and attitude per protocol for all disciplines
• Collaborate with site leadership to support staff education efforts
• Facilitates IDG meetings and provides ongoing IDG Support such as triaging incoming calls, provides caller support and assistance, and performs quality calls
• Participates in or lead quality improvement activities, including data collection and aggregation as able
• Reinforces and supports all internal and external communications
