Job summary

AccentCare, Inc. is seeking a Registered Nurse Team Director for our Hospice location in Clearwater, FL. The role requires a nurse with at least 5 years of RN experience and a minimum of 2 years in a supervisory capacity.

Bilingual is preferred. The position runs Monday–Friday, 8am–5pm, with a focus on leading the clinical team and ensuring high-quality patient care across the coverage area. The RN Team Director will supervise care team members, manage scheduling and payroll, and ensure timely

Qualifications

• ADN or higher is required.
• RN license in the practicing state(s) is required.
• CHPN certification is preferred.
• Three+ years of hospice, home health, medical surgery, oncology, or related nursing experience.
• Minimum of one year of supervisory experience.
• CPR certification preferred for clinical role.
• Must be willing to travel to all business locations.
• Compliance with state regulations where services are provided.

Responsibilities

• Supervise direct patient and family care team members with program site leadership.
• Manage scheduling, payroll and other administrative duties.
• Ensure initial assessments and required reassessments are conducted timely by qualified staff.
• Oversee IDG development and care plans for each patient and family.
• Collaborate with site leadership to monitor staff performance, productivity, and documentation.
• Support Back to Bedside and Case Management initiatives per protocol.
• Assist staff education efforts and facilitate IDG meetings.
• Participate in quality improvement activities and data collection.

Education

ADN or higher

Registered Nurse license in state(s) practicing, required

Three+ years' experience in hospice, home health, general medical surgery, oncology, or patient population-specific nursing

Hospice Registered Nurse Team Director Responsibilities:

• Supervise direct patient and family care team members, in collaboration with program site leadership
• Scheduling, payroll, and other administrative duties
• Ensure all initial assessments and required reassessments are conducted by qualified team members within required time frames, including documentation submissions, and/or synchronization
• Oversee and support the Interdisciplinary Group (IDG) in development, implementation, and review of the interdisciplinary plan of care for each patient and family
• Collaborate with program site leadership members to monitor, manage, and mentor staff in patient care activities, productivity, and documentation per related policies and protocols
• Participates and promotes Back to Bedside efforts in reinforcing Case Management skills, and knowledge and attitude per protocol for all disciplines
• Collaborate with site leadership to support staff education efforts
• Facilitates IDG meetings and provides ongoing IDG Support such as triaging incoming calls, provides caller support and assistance, and performs quality calls
• Participates in or lead quality improvement activities, including data collection and aggregation as able
• Reinforces and supports all internal and external communications
