All Metro Health Care - Corning is seeking a Per Diem RN Supervisor to support home care clients and caregivers in the Corning area. You will provide clinical oversight, conduct home visits, and mentor staff to ensure safe, high-quality care in clients' homes.

This field-based role requires 1–2 days per week, flexible visit schedules, and mileage reimbursement; valid NYS RN license and CPR are required, with at least one year of home care and supervisory experience.

Per Diem RN Supervisor – Home Care

All Metro Health Care - Corning is looking for an experienced Per Diem RN Supervisor to support our home care clients and caregivers in the Corning area. In this role, you’ll provide clinical oversight, conduct home visits, and mentor caregivers while helping clients receive safe, high-quality care in the comfort of their homes.

Pay & Position Details

Pay Per Visit:

• Supervisory/Post-Hospital Visit: $35-$60
• Reassessment: $75
• Initial Admission: $90 weekday / $125 weekend
• Schedule:Visits planned with the required presence of the patient's aide.
• Mileage reimbursement at the current federal rate

What You’ll Do

• Evaluate, educate, and support caregivers on assigned cases

This is a field-based Per diem role requiring availability 1–2 days per week.Visits are scheduled based on client and caregiver availability.
