Job summary

All Metro Health Care - Rochester is seeking a Per Diem RN Supervisor to oversee home care clients and caregivers in the Rochester area. The role includes clinical oversight, home visits, and mentoring caregivers to ensure safe, high-quality care in clients' homes.

You will provide supervision, perform assessments, and support the care team with plans of care, while maintaining compliance with standards and regulations.

Qualifications

• Licensed RN in New York State.
• CPR certification required.
• Minimum 1 year home care experience.
• Minimum 1 year supervisory or management experience.
• Valid NYS driver's license, insurance, registration, and reliable car.

Responsibilities

• Evaluate, educate, and support caregivers on assigned cases.
• Provide clinical orientation and participate in training for new nurses and caregivers.
• Conduct initial assessments and supervisory home visits.
• Assist physician in developing and updating plans of care based on client needs.
• Counsel clients and caregivers regarding nursing and care needs.
• Complete clinical progress notes and report changes in client condition.
• Review clinical records for accurate and complete documentation.

Per Diem RN Supervisor - Home Care

Rochester, NY | Flexible, Visit-Based Schedule
Office Location: 1344 University Avenue, Suite 100, Rochester, NY 14607

All Metro Health Care - Rochester is looking for an experienced Per Diem RN Supervisor to support our home care clients and caregivers in the Rochester area. In this role, you'll provide clinical oversight, conduct home visits, and mentor caregivers while helping clients receive safe, high-quality care in the comfort of their homes.

Pay & Position Details:

Pay Per Visit:

• Sup/Post Hospital: $35/$60
• Reassessment:$75
• Initial Admission:$90 weekday/ $125weekend
• Schedule:Services will be provided 2 weekdays per week during daytime hours, with one of those days conducted on-site.isits planned with the required presence of the patient's aide
• Mileage Reimbursement: Provided at the current federal rate

What You'll Do:

• Assists physician in developing a plan of treatment/care and update plans of care based on client's needs/condition.
• Counsel clients and caregivers regarding nursing and care needs
• Complete clinical progress notes and report changes in client condition
• Review clinical records for accurate and complete documentation
