Job summary

Jobtailor is seeking an experienced RN to manage a caseload of medically fragile patients in the Addison, TX area. The role includes supervising in-home skilled nursing care, coordinating Start of Care assessments, and transitioning patients from facilities to home care.

The ideal candidate will have an active RN license, at least 1 year of hands-on nursing experience, CPR certification, and willingness to supervise staff delivering home-based care to diverse patient populations.

Responsibilities

• Manage a caseload of medically fragile patients
• Manage internal casework and evaluate nursing talent development
• Perform Start of Care Assessments and Re-evaluations to transition patients from facilities to home care
• Review physicians’ orders and update care plans and documentation as needed
• Assist in interviewing, selecting, training, and validating new clinical staff
• Enhance field staff’s clinical expertise to ensure safe, high-quality nursing care
• Collaborate with the operations and business development team
• Ensure compliance with State, Federal, Local, and Aveanna policies and procedures
• Mentor and guide field staff in patient homes
• Assist with hospital discharges and coordinate start-of-care processes
• Validate nurse competencies

Requirements

• Current, good-standing RN license in the state of practice
• At least 1 year of hands-on nursing experience
• Ability to travel to patients’ homes within the designated territory
• Reliable transportation
• Valid driver’s license
• Ability to pass MVR check
• Current CPR certification (AHA or ARC)
• Willingness to supervise nurses providing in-home skilled nursing care to infants, children, adolescents, and adults
• Compliance with applicable health-related requirements, including vaccinations and testing, subject to medical or religious exemptions
