Job summary

Elara Caring is seeking a registered nurse to provide high-quality home health care across Texas. You will assess patient and family needs, plan and coordinate care, and evaluate nursing outcomes during visits.

Responsibilities include developing and adjusting care plans, communicating with supervisors and the care team, and ensuring HIPAA privacy throughout all activities. Travel up to 50% is required.

Qualifications

• Current RN license in the state.
• 1+ year of clinical nursing experience.
• Travel 50% within branch/office coverage area.
• Able to lift up to 50 pounds and assist with patient transfer.

Responsibilities

• Assess caseload and analyze physical, psychosocial, and environmental needs.
• Develop and revise care plans; coordinate with team and supervisors.
• Conduct initial and ongoing patient evaluations during visits.
• Communicate findings to Clinical Supervisor, physician, and care team.
• Ensure patient privacy and HIPAA compliance during all interactions.

Job description

• Provide high-quality home health care to patients in their preferred home setting
• Assess assigned caseload of home health care patients and families to identify physical, psychosocial, and environmental needs
• Assume primary responsibility for the caseload, including assessing, planning, coordinating, implementing, and evaluating treatment plans
• Assess physical, psychological, social, and spiritual factors influencing patient, family, and caregiver health status and incorporate data into the plan of care
• Conduct initial evaluations and re-evaluate patients’ nursing needs during each visit
• Make follow-up visits according to patient, family, and facility needs
• Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other care personnel
• Revise plans of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility, and other team members
• Align all activities with Elara Caring’s vision and Home Health leadership
• Report to the Clinical Manager or Clinical Supervisor

Requirements

• Current, unrestricted state RN license
• 1+ year experience in a clinical care setting as a nurse
• Able and willing to travel within branch/office coverage area
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Dependable vehicle
• Valid driver’s license
• Current auto insurance in accordance with state laws
• Must be able and willing to travel 50%
• Associate’s Degree or Bachelor’s Degree in Nursing is preferred
• Previous Home Health or Hospice experience is preferred
• Previous experience with HomeCare HomeBase is preferred
• Must maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule
