Job summary

Northwestern Medicine is seeking an RN to serve as Admission Process & Liaison professional at our hospital campus. The role involves clinically assessing referred patients, coordinating admissions, and liaising with internal and external teams to ensure cost-effective care plans.

Strong communication, organizational skills, and knowledge of rehab are essential. Ideal candidates have a BSN, Illinois RN license, and experience in acute or rehab settings.

Qualifications

• Bachelor’s Degree: Nursing
• Active Illinois License: RN
• Computer experience in a Windows environment
• Strong verbal communication skills, ability to work independently, and knowledge of medical terminology/pathophysiology and rehabilitation
• Effective time management and organizational skills
• Strong customer service skills
• Four to five years of recent acute or rehabilitation care experience
• Prior marketing, case management, or liaison experience

Responsibilities

• Admits and liaises with patients, families, and discharge planners; develops rounding schedule to meet physician and patient needs
• Responds to patients and families within 24 hours with results of clinical review and ongoing needs
• Collects demographic, financial, and medical information; forwards to Admission Representative
• Evaluates medical information to determine level of care and rehabilitation criteria with Medical staff
• Acts as liaison case manager with internal and external teams to provide cost-effective plans for each client
• Coordinates admissions with in-house staff, pre-certification, and transfer timing; informs staff of admission dates
• Promotes professional representation to referral sources and maintains positive relations with hospitals
• Completes and submits required reporting and documentation in a timely manner
• Attends sales, marketing, and departmental meetings
• Reports changes in targeted hospitals and action plans to Director or designee
