Job summary

Northwestern Medicine Central DuPage Hospital is seeking a registered nurse to serve in admission liaison activities within the hospital campus. The role involves clinically assessing referred patients, coordinating admissions and transfers, and working with discharge planners to ensure seamless transitions and appropriate levels of care.

Requirements include a Bachelor’s Degree in Nursing and an active Illinois RN license, with strong verbal communication, time management, and customer service

Qualifications

• Bachelor’s Degree: Nursing is required.
• Active Illinois License: RN is required.
• Computer experience in a Windows environment, or equivalent experience.
• Strong verbal communication skills, ability to work independently, and knowledge of medical terminology/pathophysiology and rehabilitation.
• Effective time management and organizational skills.
• Strong customer service skills.

Responsibilities

• Clinically assesses referred patients at targeted hospitals daily and develops a rounding schedule to meet the needs of physicians, customers, families and discharge planners.
• Responds to patients, families, and discharge planners within 24 hours with results of clinical review and ongoing needs.
• Liaises with internal/external case management to provide a cost‑effective plan for each client.
• Coordinates admission processes and transfers with in-house admission staff.
• Maintains positive, professional demeanor and communicates effectively with referral sources.
