University of Virginia Health is seeking an RN Navigator to provide specialized clinical knowledge and individualized support across the care continuum for patients, families, and caregivers. The role guides patients through evidence-based care pathways and coordinates care from diagnosis through treatment and follow-up, addressing barriers to care to improve outcomes.

Competenze

• Bachelor of Science in Nursing from an accredited nursing program.
• 1 year of registered nursing experience required, 3 years of related and relevant RN experience strongly preferred.
• Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia; AHA Health Care Provider BLS certification required; Specialty board certification strongly preferred.

• Demonstrates professional nursing practice and quality achievement; lifelong learning and advocacy.
• Manages complex care and high-risk patient populations; alleviates barriers to care.
• Triages new patient referrals and assesses patient needs, goals, and preferences; integrates into the care plan.
• Coordinates multidisciplinary care and expedites access to treatments including diagnostic testing, surgery, and follow-up care.
• Serves as a clinical advocate, educator, and liaison among patients, families, and providers.
• Provides culturally competent education on diagnosis, treatment, side effects, and posttreatment care.
• Facilitates communication between care teams and supports participation in clinical trials.
• Identifies programmatic and quality initiatives and tracks outcomes along the care pathway.
