The role requires an Associate or Bachelor's degree in nursing, active RN licensure for the state, CPR certification, and 1+ year of RN experience in home health or hospice plus RN management experience. A driver's license and transportation are required.

Qualifications

• Associate or bachelor’s degree in nursing.
• Current RN license valid for the state(s) of assignment.
• Current CPR certification.
• 1+ years of clinical RN experience in home health or hospice.
• 1+ years of RN management experience.
• Driver's license and reliable transportation.

Responsibilities

• Recommends employment decisions for staff to the director of operations.
• Responsible for overall quality of patient care and clinical performance of nurses and aides.
• Organizes clinical operations for the care center.
• Reviews requests for services and determines patient eligibility/suitability for home care services.
• Oversees scheduling.
• Oversees payor verification and precertification requirements.
• Reviews documentation and resolves missing/incomplete/untimely entries.
• Assists clinicians in establishing Plan of Care and priorities.
• Regularly evaluates patient needs and revises the plan of care.
• May provide direct patient care in emergencies.
• Participates in on-call process.
• Assists with staff training to ensure quality care and fiscal responsibility.
• Participates in investigation/resolution of concerns.
