Qualifications

• Associate or bachelor’s degree in nursing.
• Current RN license for the assigned states.
• Current CPR certification.
• 1+ years of clinical RN experience in home health or hospice.
• 1+ years of RN management experience.
• Driver's License and access to reliable transportation.

Responsibilities

• Provide leadership, supervision, and coordination of therapy services.
• Oversee clinical operations and quality of patient care.
• Review service requests and determine patient eligibility for home care.
• Coordinate referrals, scheduling, and patient visits.
• Oversee payor verification and precertification requirements.
• Review staff documentation and ensure accuracy and timeliness.
• Assist clinicians in establishing Plan of Care (POC) with priorities.
• Evaluate patient needs and revise the plan of care as needed.
• May provide direct patient care in emergencies.
• Participate in on-call processes and staff training.
• Address patient/family/physician concerns and resolutions.
