Granite Peak Home Health in Montana seeks an experienced Clinical Manager (RN) to oversee patient care in the designated region. You will supervise staff, coordinate referrals, and ensure high-quality, compliant home health services.

This full-time role requires an active Montana RN license, at least 2 years of home health experience, and proficiency with OASIS-E. Strong leadership and budgeting skills are a plus as you support CMS guidelines and PDGM compliance.

Qualifications

• Bachelor’s degree in nursing is required.
• Current Montana RN license.
• Minimum 2 years nursing experience in home health.
• Proficiency with OASIS-E assessment.
• Knowledge of CMS PDGM and Montana DPHHS guidelines.
• Experience with electronic health records (WellSky) and web-based systems.

Responsabilités

• Oversee all patient care within the designated region.
• Provide clinical supervision and staff management.
• Coordinate referrals and develop care plans.
• Lead weekly IDT and leadership meetings.
• Ensure regulatory compliance and quality assurance.
• Assist in budgeting and monitoring census, staffing, and expenses.
• Ensure accurate patient documentation and reporting.
• Maintain supplies and equipment.
