Join to apply for the RN Clinical Manager Home Health role at Gallin Associates

We are seeking an experienced and highly motivated Clinical Manager to lead and oversee clinical operations in a home health setting.

Schedule: Full Time, 8am - 5pm

Note: This position does not have a team of clinicians reporting to them.

Responsibilities
  1. Clinical Leadership and Compliance: Supervise day-to-day clinical and office operations. Be available to staff and patients during hours of operation. Ensure services comply with professional standards, agency policy, and regulatory requirements. Serve as backup support for PCM, CSS, and lead in the absence of the Territory – Administrator.
  2. Clinical Oversight of Patient Care: Coordinate referrals and patient care with an interdisciplinary team. Develop, implement, and update individualized care plans. Evaluate clinical care quality, collaborate on patient transfers/discharges, review discharge data, and manage KPIs and workflow.
  3. Clinical Oversight of Personnel: Ensure clinicians and contract staff are trained, competent, and supervised. Assign patients and tasks based on staff competence. Provide ongoing coaching and mentoring.
  4. Quality and Compliance: Act as QAPI owner, participate in QA activities, review incidents, and implement corrective actions. Manage accreditation readiness, audits, and HHCAHPS distribution. Oversee emergency preparedness planning.
Qualifications
  1. Bachelor’s degree in nursing, healthcare administration, or related field; Master’s preferred.
  2. Current nursing or clinical licensure.
  3. Experience in HH leadership and OASIS.
  4. Knowledge of QAPI and survey readiness (a plus).
  5. Proven clinical managerial experience.
  6. Strong understanding of healthcare regulations and compliance.
  7. Excellent leadership, organizational, and communication skills.
  8. Ability to analyze patient data and apply evidence-based practices.
  9. Proficiency in EHR systems and healthcare technology.
