We are seeking an experienced and highly motivated Clinical Manager to lead and oversee clinical operations in a home health setting.

Schedule: Full Time, 8am - 5pm.

Would not have a team of clinicians reporting to them.

Responsibilities
  1. Clinical Leadership and Compliance: Supervised day-to-day clinical and office operations. Available to staff and patients during hours of operation. Ensures services comply with professional standards, agency policy, and regulatory requirements. Serves as a backup support for PCM, CSS, and leads in the absence of the Territory – Administrator.
  2. Clinical Oversight of Patient Care: Coordinates referrals and patient care with an interdisciplinary team. Ensures development, implementation, and updating of individualized care plans. Evaluates clinical care quality and collaborates with clinicians and physicians regarding transfers or discharges. Reviews discharge assessments, manages KPIs, workflow, and guides clinicians on Emergency Care Protocols.
  3. Clinical Oversight of Personnel: Ensures clinicians and contract staff have required training and supervision. Assigns patients and tasks based on demonstrated competence. Provides ongoing coaching and mentoring.
  4. Quality and Compliance: Manages QAPI activities, reviews incidents, and implements corrective actions. Oversees accreditation, audits, and survey readiness. Manages HHCAHPS and emergency preparedness planning.
Qualifications
  • Bachelor’s degree in nursing, healthcare administration, or related; Master’s preferred.
  • Current nursing or clinical licensure.
  • Experience in HH leadership and OASIS.
  • QAPI/Survey readiness experience is a plus.
  • Proven managerial experience in a clinical setting.
  • Strong knowledge of healthcare regulations, compliance, and quality assurance.
  • Excellent leadership, organizational, and communication skills.
  • Ability to analyze patient data and implement evidence-based practices.
  • Proficient in EHR systems and healthcare technology.
