Job summary

BAYADA Home Health Care in Cranford, NJ is seeking an experienced RN to fill the role of RN Clinical Manager in our Medicare Certified Senior Living home health office.

You will lead a multi-disciplinary team, oversee referrals, coordinate in-services, review documentation including OASIS, and ensure high-quality patient care while supporting business growth.

This full-time role offers stable employment with opportunities for growth and a schedule Monday–Friday 8:30 am–5:00 pm.

Qualifications

• RN with 2+ years of experience in nursing leadership or management.
• Medicare Certified Home Care experience.
• Strong understanding of Medicare Conditions of Participation.

Responsibilities

• Lead, inspire and coordinate care provided by a multi-disciplinary team.
• Direct and manage referrals, and promote BAYADA’s services.

Job description

BAYADA Home Health Care is seeking a Registered Nurse (RN) to fill the position of RN Clinical Manager for our Cranford, NJ Medicare Certified Senior Living home health office. This office provides best-in-class care to adult and geriatric clients living in Senior Living Communities.

Responsibilities for a Nurse Manager:

• Lead, inspire and motivate others to provide exceptional care.
• Directing and coordinating the care provided by a multi-disciplinary team of home health care professionals.
• Processing incoming referrals.
• Develop and maintain relationships with referral sources, physicians, hospitals, and community partners to promote BAYADA’s services and support business growth.
• Management and mentoring of Clinical Associates and field staff
• Facilitate new hire orientation of field staff
• Documentation review, including OASIS
• Monitoring quality assurance
• Managing staff productivity, and overall case management.
• Visiting clients as necessary.
• Coordinate and/ordeliver in-services for field staff.

Qualifications for a Clinical Nurse Manager:

• Registered Nurse (RN) with 2or more years of experience as an RN
• Recent Medicare Certified Home Care experience.
• Comprehensive knowledge of the Medicare Conditions of Participation.
• Thorough understanding of OASIS documentation and ICD-10 coding.
• Strong management and leadership background
• Excellent organizational, interpersonal and communication skills.
