Job summary

BAYADA Home Health Care in Cranford, NJ is seeking an experienced Registered Nurse (RN) to fill the RN Clinical Manager role for our Medicare Certified Senior Living home health office. You will lead a multi-disciplinary team, coordinate care, review OASIS documentation and referrals, mentor staff, and ensures quality and productivity while visiting clients as needed.

• Registered Nurse (RN) with 2+ years of RN experience and Medicare home care familiarity.
• Strong knowledge of Medicare Conditions of Participation and OASIS.
• Experience leading a multi-disciplinary care team is preferred.

Responsibilities

• Lead, direct, and mentor a multi-disciplinary home health team.
• Direct care coordination, referrals processing, and relationship-building with referral sources.
• Oversee documentation review including OASIS and maintain quality assurance.
• Manage staff productivity and perform client visits as needed.
• Coordinate in-services and staff onboarding.

BAYADA Home Health Careis seeking a Registered Nurse (RN) to fill the position of RN Clinical Manager for our Cranford, NJ Medicare Certified Senior Living home health office. This office provides best-in-class care to adult and geriatric clients living in Senior Living Communities.

Responsibilities for a Nurse Manager:

• Lead, inspire and motivate others to provide exceptional care.


• Directing and coordinating the care provided by a multi-disciplinary team of home health care professionals.


• Processing incoming referrals.


• Develop and maintain relationships with referral sources, physicians, hospitals, and community partners to promote BAYADA’s services and support business growth.


• Management and mentoring of Clinical Associates and field staff


• Facilitate new hire orientation of field staff


• Documentation review, including OASIS


• Monitoring quality assurance


• Managing staff productivity, and overall case management.


• Visiting clients as necessary.


• Coordinate and/or deliver in-services for field staff.

Qualifications for a Clinical Nurse Manager:

• Registered Nurse (RN) with 2or more years of experience as an RN


• Recent Medicare Certified Home Care experience.


• Comprehensive knowledge of the Medicare Conditions of Participation.


• Thorough understanding of OASIS documentation and ICD-10 coding.


• Strong management and leadership background


• Excellent organizational, interpersonal and communication skills.
