• Develop and update individualized plans of care based on patient needs.
• Monitor patient conditions and communicate changes to care team.
• Provide guidance to RNs, LPNs, and caregivers.
• Maintain accurate clinical documentation per policies and regulations.
• Address clinical questions and escalations; support quality and safety.
