Job summary

All Metro Health Care, a Modivcare Personal Care Service, is seeking a Full-Time RN Clinical Manager in Fort Lauderdale, FL. The role combines clinical leadership with hands-on nursing oversight in patients' homes, with a primary field-based schedule and occasional office tasks.

You will perform nursing assessments, develop care plans, and guide RNs/LPNs and caregivers while ensuring high-quality, compliant home care across the service territory.

Qualifications

• Current Florida RN license in good standing.
• Experience delivering home health or home care services.
• Strong clinical assessment and communication skills.
• Ability to travel throughout Fort Lauderdale for patient visits.
• Comfort working independently in a primarily field-based environment.

Responsibilities

• Conduct nursing assessments, reassessments, and supervisory visits in patients' homes.
• Provide clinical oversight of home care services, including the agency's waiver program.
• Develop, review, and update individualized plans of care based on patients' needs.
• Monitor patients' conditions and communicate changes to the appropriate care team members.
• Provide clinical guidance and support to RNs, LPNs, and caregivers.
• Collaborate with branch leadership and the care coordination team to support continuity of care.
• Maintain accurate clinical documentation in accordance with company policies and applicable regulations.
• Address clinical questions, patient concerns, and escalations as needed.
• Support clinical quality, patient safety, and effective coordination of services.
