Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) to fill the position of Clinical Nurse Manager for our Lebanon, MO Habilitation office. This office provides best in class care to clients of all ages throughout the Rolla area.

As Clinical Manager you will lead, develop care plans, supervise habilitation staff, conduct competencies, and collaborate with clients, families, and case managers to meet individualized goals.

Qualifications

• RN licensed nurse with leadership capability.
• Ability to lead, inspire and motivate others to provide exceptional care.
• Experience developing and overseeing care plans and their execution.

Responsibilities

• Supervise the habilitation tech staff in the field.
• Conduct competencies on new and tenured staff.
• Collaborate with the client, family, Case Managers, Hab Techs and other teams to implement and evaluate client care needs and individual goals identified in the Person Centered/Individual Support Plan
• Make regular client visits to evaluate progress and monitor service delivery.
• Evaluate progress of goals identified in the Plan.
• Maintain ongoing communication with Case Managers/Care Coordinators
• Carry out supervision of Habilitation Technicians according to policy and procedure and state regulations

Carry out supervision of Habilitation Technicians according to policy and procedure and as mandated by state regulation
