Job summary

Our client is seeking an experienced RN Clinical Liaison to drive referral growth and coordinate patient transitions from acute care facilities to home infusion services.

This role combines clinical nursing expertise with business development and relationship management, working closely with physicians, discharge planners, case managers, and healthcare facilities to develop referral partnerships and ensure seamless patient transitions.

Qualifications

• Active, unrestricted RN license required.
• Minimum 2 years infusion healthcare experience.
• Sales, business development, or referral development experience preferred.
• Experience working with physicians, discharge planners, case managers, and healthcare facilities.
• Strong relationship-building, communication, and presentation skills.

Responsibilities

• Generate new referrals and grow territory via relationships and accounts.
• Build partnerships with physicians, discharge planners, case managers, and healthcare facilities.
• Identify patients who may benefit from home infusion services.
• Coordinate patient transitions from acute care to home infusion.
• Collaborate with healthcare teams on care plans and discharge planning.
• Educate referral sources on infusion services and clinical capabilities.
• Serve as clinical resource for partners, patients, and internal teams.
• Manage territory activity, referrals, and follow-up.
• Support patient onboarding and transitions to home-based care.
