Job summary

Avow is seeking a Registered Nurse to drive referral growth within a defined Florida territory, facilitating transitions from hospital to home and collaborating with case managers, discharge planners, and physicians. You will educate admissions staff and help patients access Avow services across assigned facilities and beyond.

The role requires a Florida RN license, CPR certification, and strong relationships with healthcare partners.

Qualifications

• Graduate from an accredited school of nursing.
• BSN preferred and two years of experience in an acute care setting; one year of home care and/or hospice experience preferred.
• Current Registered Nurse license in Florida.
• Florida driver’s license and insured vehicle.
• CPR certification required upon hire.
• CHPN certification preferred.

Responsibilities

• Assess patients to determine appropriateness of Avow services.
• Identify payment options and assist with financial applications as needed.
• Coordinate referral-to-admission process for patients referred to Avow.
• Document accurately and timely in the organization’s CRM.
• Communicate with physicians, case managers, and families to support care planning.
• Build relationships with case managers, social workers, and other care providers.
• Travel to patient homes and facilities for evaluations and adheres to driving requirements.
