Mass General Brigham Home Care is seeking a Clinical Manager to lead a nursing team, oversee admissions, and ensure safe, timely, and high-quality home care. You will bring your nursing expertise, leadership, and clinical judgment to support clinical teams and improve care delivery across the home setting.

This hybrid leadership role offers remote work flexibility with on-site presence at our Chelsea or Beverly, MA offices.

Qualifications

• Bachelor's degree in Nursing required; Master's degree preferred.
• Current Registered Nurse (RN) licensure in Massachusetts required.
• Three years of nursing experience required.
• Leadership experience strongly preferred.
• Strong clinical judgment, communication, and collaboration skills.
• Ability to support clinical teams, navigate complex admissions, and promote high-quality, compliant patient care.

Responsibilities

• Overseeing home health admissions, including referral review and Start of Care coordination.
• Ensuring timely, accurate, and compliant admission documentation, including OASIS oversight.
• Serving as a clinical decision-maker for complex or high-risk admissions.
• Coaching and developing admissions and intake staff through training and feedback.
• Monitoring admission metrics and identifying workflow, quality, and efficiency improvements.
• Collaborating with referral partners and internal teams to support smooth transitions of care.
• Participating in management responsibilities, including holiday coverage and on-call rotation.
