Job summary

CenterWell Home Health in Massachusetts is seeking a Registered Nurse Clinical Manager to lead a team of clinicians delivering home-based care. You will oversee clinical operations, ensure regulatory compliance, and support care planning to help patients reach independence.

Responsibilities include staff development, admissions review, documentation quality, and collaboration with physicians and caregivers. Some direct patient care may be required, with Branch Director coverage in absence.

Qualifications

• Current state RN license.
• CPR certification.
• Valid driver's license and reliable transportation.
• Two years as a Registered Nurse with at least one year of management experience in a home care, hospice or equivalent environment.

Responsibilities

• Oversee clinical operations for the location, including patient care delivery, staff management, and regulatory compliance onsite.
• Review referrals, determine admission appropriateness, assign clinicians, and ensure Plans of Care meet patient needs.
• Guide and educate clinicians; support care planning and clinical decision-making; be available during hours for clinical support.
• Ensure clinical documentation, audits, and billing meet Medicare and payer standards; monitor case management quality.
• Participate in hiring, training, performance evaluation, coaching, and corrective actions for clinical staff.
• Conduct ongoing staff education based on documentation reviews and performance improvement data.
• Coordinate communication among physicians, team members, and caregivers to support care coordination and discharge planning.
• Participate in quality improvement, data tracking, budgeting, marketing, and community relationship development.
• Provide direct patient care on rare occasions and act as Branch Director in absence.
• Perform additional tasks to support clinical operations and organizational goals.
