Emblem Home Health in Phoenix, AZ is seeking an RN Clinical Intake Coordinator for a Friday–Monday shift, four 10-hour days. The role includes patient intake, referral coordination, and admissions, with Saturdays and Sundays eligible for remote work.

You will verify insurance, ensure timely admissions, build relationships with physicians and hospitals, and track referral data. Arizona RN license and CPR are required; home health experience is a plus, and strong communication is essential.

Qualificações

• Must hold an active Arizona RN license.
• Home health experience is a plus.
• Experience in patient intake, admissions or care coordination preferred.
• Excellent communication, customer service and relationship-building skills.
• Knowledge of healthcare regulations and business operations.
• Current CPR certification required.

Responsabilidades

• Oversee all daily patient intake and referral operations.
• Manage the full intake process, including insurance verification and authorization.
• Ensure compliance with all state, federal, and payer requirements.
• Coordinate timely patient admissions in accordance with company policies.
• Build and maintain strong relationships with physicians, hospitals, and referral sources.
• Ensure smooth patient transitions to home care, including care coordination and education.
• Track and report referral data, trends, and performance metrics.
• Promote and maintain a positive reputation in the community as a high-quality provider.
• Identify opportunities to improve services and patient access to care.
