Job summary

Kettering Home Care in the United States is seeking a Registered Nurse (RN) Clinical Field Staff Liaison to support field clinicians and ensure quality of home health delivery. The role involves travel to field sites and collaboration with physicians, patients and families.

The ideal candidate has 3+ years RN experience in acute care, current state licensure, and optional home health background. Training occurs in-office for 90 days, with a potential hybrid schedule (3 days remote, 2 in-office).

Qualifications

• Registered Nurse with current license in the state of employment.
• Compact licensure required to be obtained within 30 days for additional support.
• Minimum three years of RN experience in an acute care setting.
• One year of home health experience preferred.
• Ability to communicate effectively with patients, families, physicians and coworkers.
• Detail-oriented, calm under pressure, team player.

Responsibilities

• Serve as a resource for field staff, referral sources, physicians, patients and families on clinical issues.
• Facilitate calls to and from field staff and escalate issues to management.
• Travel to field sites and work in the field with staff and patients as needed.
• Maintain positive relationships with referral sources and facilities.
• Support monthly Case Manager, Nurse and Therapy meetings.
• Ensure Start of Care compliance and quality/safety of home health services.
• Monitor Home Health Aide supervisory visits and outcomes.
• Review clinical documentation per agency protocol and Medicare/Federal guidelines.
