Job summary

Hackensack Meridian Health is seeking a Clinical Coordinator to collaborate with the Clinical Program Manager and subspecialty teams, coordinating care for complex patients and families. The RN will provide professional nursing care, patient education, and act as a liaison between families and subspecialty providers to ensure coordinated, high-quality care.

The role requires a BSN, 3–5 years of acute/primary care nursing, strong communication, and proficiency with Microsoft Office/Google Suite;

Qualifications

• BSN required for RN licensure and clinical practice.
• 3–5 years acute/primary care nursing experience in a specialty setting preferred.
• Proficiency with Microsoft Office and Google Suite for documentation and care coordination.

Responsibilities

• Identify, support and develop programs to meet needs of complex patients and families.
• Act as liaison between families and subspecialty team to coordinate care and education.
• Provide professional nursing care and disease-specific education to patients and staff.
