Job summary

Loretto Health & Rehabilitation seeks a Clinical Coordinator, RN to coordinate clinical practice and care for residents in a long term care and short term rehab setting. Under the Unit Nurse Manager, the Clinical Coordinator acts as liaison between residents, families, medical providers and staff.

Conducts patient assessments and evaluates nursing care effectiveness. Salary range is $84,323 - $94,702 annually with a Monday–Friday schedule.

Qualifications

• 1+ year nursing experience preferred.
• Current RN NYS license.
• Established patient assessment skills.
• Knowledge of best-practice standards.
• Skilled to respond to emergent situations.
• Ability to analyze problems, plan resolution strategies, implement solutions and evaluate outcomes.
• Exceptional interpersonal and customer service skills.
• Proficiency in speaking, listening, reading and writing English.

Responsibilities

• Oversee the delivery of Quality Patient Care.
• Compliance with NYSDOH and CMS regulations and facility policies and protocols.
• Orients, trains and mentors new employees.
• Provides health training to patients and their families.
• Assists in gathering and analyzing data for QAPI.
• Acts as primary liaison with medical providers.
