Job summary

Logan Regional Medical Center is seeking an RN-Clinical Coordinator to coordinate multidisciplinary care across ICU, Medical, Surgical, and Ancillary units while supporting quality initiatives in an acute care setting.

You will collaborate with nursing staff, physicians, and other departments to assess, plan, implement, and evaluate patient care, ensuring smooth discharge planning and staff education. Strong regulatory knowledge and EMR proficiency are essential.

Qualifications

• ADN is required.
• BSN preferred.
• Experience coordinating care across units and with multidisciplinary teams is expected.

Responsibilities

• Coordinate care across ICU, Medical, and Surgical Units.
• Collaborate with interdisciplinary teams to plan, implement, and evaluate care.
• Provide clinical guidance to nursing staff on procedures, protocols, and policies.
• Participate in quality improvement initiatives and regulatory compliance.
• Conduct daily patient rounds to assess needs and care quality.
• Assist in discharge planning and patient education for follow-up care.
• Develop educational programs to enhance competencies and promote best practices.
• Monitor regulatory guidelines and ensure EMR documentation accuracy.
• Document patient care activities in EMR and other clinical systems.
