Safe Life Home Health Care is seeking an office-based Clinical Case Manager to support referral intake, triage, coordination of patient care, and communication with patients, families, referral sources, and the clinical team. This role is ideal for an organized RN comfortable managing a high volume of cases in a fast-paced home health setting.

The position covers Lombard and Chicago branches, with compensation of $35-$40 per hour.

Qualifications

• Active RN license in good standing.
• Experience in home health, intake, referral management, or clinical triage.
• Strong communication and multitasking skills.

Responsabilités

• Oversight of patient care services and staffing assignments.
• Triage referrals and coordinate admission readiness.
• Coordinate care with hospitals, physicians, discharge planners and intake teams.
• Provide ongoing staff training and performance reviews.
• Ensure compliance with home health regulations and company policies.
• Support Lombard and Chicago branches as needed.
