Job summary

BAYADA Home Health Care is seeking a Clinical Manager in the Assistive Care/Home Care program to lead clinical oversight for home-based clients in Arizona. This RN role collaborates with the office team to ensure safe, compliant, high-quality care and optimal client outcomes.

The Clinical Manager will assess clients, develop and monitor care plans, and supervise field staff while ensuring regulatory adherence and documentation accuracy.

Qualifications

• Graduate of an accredited school of nursing; RN license in good standing in the state of Arizona.
• Minimum of two (2) years of recent clinical experience, preferably in home care, med-surg, LTC, or related setting.
• Open to new grads and/or newly licensed RN's.

Responsibilities

• Conduct comprehensive client assessments in the home and complete the care planning process, including initial and ongoing reassessments.
• Develop, implement, and update individualized care plans reflecting client goals, status, risks, and social determinants.
• Provide ongoing clinical and case management oversight for an assigned caseload in line with BAYADA standards.
• Collaborate with physicians and other health care providers to coordinate care and document verbal orders.
• Conduct clinical visits and supervisory visits, documenting changes and elevating complex cases as needed.
• Provide clinical supervision and development for home health aides and field staff.
• Support caregiver-client matches and client satisfaction through effective communication.
