Job summary

Beartooth Billings Clinic is seeking a Clinic Case Manager in Red Lodge, MT. This full-time, non-exempt role focuses on coordinating patient care, linking clients with healthcare and community services, and ensuring timely access to services.

Responsibilities include assessment, planning, advocacy, education, and comprehensive documentation for a designated caseload. The ideal candidate is a graduate of an accredited nursing program with MT RN licensure, CPR certification (or ability to obtain),

Qualifications

• Graduate of an accredited school of nursing.
• Current Montana RN license or multi-state license with compact state authority.
• Current Healthcare Provider CPR certification or ability to obtain within 90 days of hire.
• 3 years of experience in a medical setting or equivalent combination of education and experience.
• Equivalent combination of education and experience will be considered.
• Cerner experience preferred.

Responsibilities

• Assess, plan, coordinate and advocate for patient care.
• Coordinate care across outpatient and inpatient services as needed.
• Educate patients and families and document care accurately.
• Manage an assigned patient caseload with ongoing monitoring and coordination.
