Beartooth Billings Clinic is seeking a Clinic Case Manager in Red Lodge, MT. This full-time, non-exempt role focuses on coordinating patient care, linking clients with healthcare and community services, and ensuring timely access to services.

Qualifications

• Graduate of an accredited school of nursing.
• Current Montana RN license or multi-state license with compact state authority.
• Current Healthcare Provider CPR certification or ability to obtain within 90 days of hire.
• 3 years of experience in a medical setting or equivalent combination of education and experience.
• Equivalent combination of education and experience will be considered.
• Cerner experience preferred.

Responsibilities

• Assess, plan, coordinate and advocate for patient care.
• Coordinate care across outpatient and inpatient services as needed.
• Educate patients and families and document care accurately.
• Manage an assigned patient caseload with ongoing monitoring and coordination.

Status: Full Time – Non-Exempt

Reports to: Clinic Manager

Purpose and Scope of Position

This position is responsible for the direct provision of services that assist patients with the achievement of their highest level of wellness. The Clinic Case Manager is focused on client centered services that link clients and other family members with health care, psychosocial and other home, and community-based services to ensure timely, coordinated access to medically appropriate levels of health and support services and continuity of care. Is responsible for assessment, planning, coordination, advocacy, education, monitoring and accurate, timely documentation in all program areas. This position is accountable for a designated patient caseload/population and plans effectively in order to meet patient needs. Is responsible for coordinating the care and services of identified patient populations for an extended period across the continuum of care for outpatient services and assisting with inpatient services as needed.

Job Requirements

1. Required Qualifications

• Graduate of an accredited school of nursing
• Current Montana RN License or current multi-state license with the ability to practice in all compact states.
• Current Healthcare Provider CPR certification or the ability to obtain within 90 days of hire
• 3 years of experience in a medical setting related to the above tasks, knowledge, skills and abilities or an equivalent combination of education and experience
• Or an equivalent combination of education and experience relating to the above tasks, knowledge, skills and abilities will be considered
