Job summary

Avera Sacred Heart Pavilion in Yankton, SD is seeking a Registered Nurse to support patient care navigation in the primary care clinic. The RN will manage care for the patient panel, promote education and self-management, and help develop care management processes to ensure timely and safe health care delivery.

The role involves coordinating care for high-acuity patients, collaborating with physicians and clinic staff, and reporting on quality measures within the medical home model.

Qualifications

• Active RN license in the state of practice.
• 1–3 years of clinic nursing experience.
• 4–6 years of case management and/or homecare experience.

Responsibilities

• Support patient care navigation in the clinic setting.
• Manage care for patients in the primary care practice to promote education, self-management, and timely health care delivery.
• Develop and monitor care management processes and support primary clinical teams.
• Coordinate care for high-acuity patient populations.
• Collaborate with physicians, providers, and practice staff in identifying appropriate patients for care management.
• Provide follow-up planning and documentation to ensure compliance with recommendations.
