The position is PRN with a schedule of at least one shift per week, typically Monday through Friday, 8:00 am to 5:00 pm.

Qualifications

• RN licensure active in the state of practice.
• 1–3 years of relevant nursing experience preferred.
• Must be able to work the hours specified (Mon–Fri 8am–5pm; PRN with at least one scheduled shift per week).

Responsibilities

• Collaborates with the multidisciplinary care team to develop an established treatment plan.
• Facilitates transitions between levels of care to promote optimal patient outcomes.
• Identifies community resources for non-covered benefits (medications, nutrition, transportation, DME).
• Provides patient care and assists providers as needed.
• Completes documentation, obtains insurance prior authorizations, and coordinates scheduling of procedures and treatments.
• Performs patient and family education on healthcare needs and follow-up care.
• Attends discharge planning meetings to coordinate post-procedure needs when required.
