Job summary

Family Health Center of Worcester, Inc. seeks an RN in the Office based Addiction Program and Chronic Pain Program as part of the Primary Care Team.

The RN coordinates care for high‑risk patients, provides direct patient care, and supports transitions across health settings to ensure patient-centered, effective care. The ideal candidate holds an Associate Degree in Nursing with MA RN licensure, BLS, and preferred ACLS.

Qualifications

• Requires RN licensure in MA.
• BLS required; ACLS preferred.
• Two years’ experience in case/disease management or related ambulatory care settings.
• Experience with EMR systems,NextGen preferred.

Responsibilities

• Coordinate care for high‑risk patients within the practice.
• Provide direct patient care and care management services.
• Support transitions across health settings and multiple providers.
• Uphold patient‑centered, culturally competent care and maintain HIPAA compliance.
