Job summary

Marathon Health seeks a Chronic Condition Manager to educate, counsel, and coach patients on prevention and management of chronic diseases through in-person, virtual, and telephonic formats. The CCM may coordinate efforts among client vendors to ensure cohesive services across locations.

The role requires RN licensure with a minimum of 3 years in chronic disease management; CDCES certification is strongly preferred, with a patient-centered focus and collaborative teamwork across the health

Qualifications

• RN with at least 3 years’ experience in chronic disease management.
• Certified Diabetes Care and Education Specialist (CDCES) certification is strongly preferred.

Responsibilities

• Facilitate chronic condition management and prevention programs across multiple modalities (face-to-face, virtual, telephonic).
• Educate, counsel, and coach patients to prevent and manage chronic diseases.
• Develop individualized interventions to improve health outcomes.
• Coordinate with healthcare team and vendors to ensure cohesive services for client populations.
• Document thorough evaluations and outcomes in the EMR and monitor program effectiveness.
