This role focuses on coordinated, patient-centered care delivery, emphasizing chronic condition management, patient education, and collaboration with interdisciplinary teams to improve outcomes and reduce care gaps. Key Responsibilities

• Deliver comprehensive care management services across diverse patient populations
• Conduct patient assessments and develop individualized care plans
• Support chronic disease management (e.g., diabetes, hypertension, CHF, COPD)
• Provide patient education on conditions, medications, and self-management strategies

Required Qualifications

• Active Registered Nurse (RN) Compact License (NLC)
• Minimum of 2 years’ experience in care management, case management, or chronic disease management

Preferred Qualifications

• Experience in:
  • Chronic care management programs (CCM)
  • Transitional care management (TCM)
  • Remote patient monitoring (RPM)
  • Telehealth or virtual care delivery
  • Strong background in patient education and engagement
