The role involves rounds, documentation, medication administration, physician rounds, and adherence to facility policies, with focus on resident well‑being and timely communication with families or contacts.

Qualifications

• Has completed RN program at an accredited institution.
• Must be an active Registered Nurse in Texas in good standing.
• Experience working in a skilled nursing facility is preferred.

Responsibilities

• During rounds observes, assesses, documents, and reports resident condition/changes.
• Assures implementation of care plans as directed.
• Administers medications as prescribed.
• Conduct rounds with physician and record visits in clinical records.
• Perform narcotic reconciliation with incoming and outgoing shifts.
• Identify significant changes in resident condition and take action per policy.
• Oversees and monitors aides.
• Initiate and complete Incident and Accident Reports per policy.
• Discharge residents per policy.
• Must complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident condition.
• Communicate with resident’s primary contact when updates are requested or condition changes occur.

Education

RN program completed at accredited vocational school, college, or university
