Job summary

Remington-Transitional-Care-Center is seeking an RN Charge Nurse to ensure high-quality care and oversee care plans. You will administer medications, participate in rounds with physicians, and document changes in resident conditions.

The role includes supervising aides, handling incident reports, and coordinating discharge processes as needed. The ideal candidate has completed an RN program and holds an active Texas RN license; SNF facility experience is preferred.

Qualifications

• Must have completed an RN program at an accredited school or university.
• Must be an active Registered Nurse in Texas in good standing.
• Experience in a skilled nursing facility is preferred.

Responsibilities

• Observe, assess, document, and report resident condition during rounds.
• Administer medications as prescribed and maintain accurate records.
• Conduct rounds with physician and record visits in clinical records.
• Perform narcotic reconciliation with incoming/outgoing shifts.
• Identify changes in resident condition and respond per policy.
• Oversee and supervise aides and nursing assistants.
• Initiate and complete Incident and Accident Reports.
• Discharge residents per facility policy.
• Complete Nursing Assessments for physician calls before contacting the physician.
• Communicate with the resident's designated contact about status or changes.
