Job summary

myPlace Health is seeking a Clinical Documentation Integrity (CDI) Specialist to ensure accurate provider documentation across the participant population. You will collaborate with physicians, nurse practitioners, and care teams to reflect clinical complexity and CMS-HCC capture.

Hybrid role with primary location in Compton, CA. The CDI Specialist reviews records, develops provider queries, and leads education to improve documentation quality in a value-based care setting.

Qualifications

• Active, unrestricted RN or NP license in California.
• Minimum of three years of clinical experience in outpatient/ambulatory settings.
• Ability to interpret, analyze, and synthesize clinical information across complex chart data.
• Foundational knowledge of ICD-10-CM coding and its relation to documentation requirements.
• Strong written and verbal communication; engaging with clinicians and care teams.

Responsibilities

• Conduct chart reviews to identify documentation gaps and CMS-HCC opportunities.
• Synthesize information from EMR, labs, imaging, medications, and notes to support accurate documentation.
• Review longitudinal trends to reflect disease progression and patient complexity.
• Formulate compliant provider queries with objective evidence.
• Educate and partner with physicians and care teams to improve documentation quality.
• Identify opportunities in real-time during interdisciplinary workflows.
• Track documentation metrics and support audit readiness.
• Develop provider-facing CDI resources and templates.
